CMS Mandatory Medical Forms
Students that require medical attention during school hours are required by CMS to have proper documentation. This documentation allows school and medical staff to treat your child. Please note that these medical forms must be filled out by BOTH parent and physician.
Scans or clear photos of completed Medical Forms must be emailed directly to BOTH Gigi Atkinson (School Registrar) and Cassi King (School Nurse).
